Conduct a short After Action Review after a railway disruption without turning it into blame or storytelling.
The review is not finished when everyone agrees what went wrong; it is finished when the next shift knows what changes. Expected Start with the plan or standard. What did the timetable, rule, checklist, recovery target, or communication protocol expect to happen? Actual Separate fact from interpretation. Use times, calls, decisions, and observed passenger or train impacts. Difference And Change Ask why the gap appeared, then decide what to sustain or adjust. Keep the output small enough that someone can actually use it on the next shift.
