Make One Human Accountable for Every Agent Run
Define clear accountability for an AI agent run before any agent starts work.
The teach RACI prevents a common agent-orchestration failure: many capable actors, no clear owner. Before starting a run, write the work as a small responsibility assignment matrix. Responsible: who runs the workflow and packages the output. Accountable: who accepts or rejects the result. Consulted: whose expertise or system evidence must shape the work. Informed: who receives the decision, not every draft. The key design rule is one Accountable human per outcome.
