Write a specific hand hygiene observation note that identifies the moment, behavior, and immediate feedback.
An audit note reads only: missed HH. The unit cannot use it to improve behavior. Moment + observed touch + hand hygiene behavior + feedback = coachable compliance data A vague note turns a specific workflow miss into a generic score, so staff cannot see what to change. Name the moment Identify the WHO indication: Moment 1, 2, 3, 4, or 5. The moment explains the transmission direction and prevents generic feedback.
