Evaluate whether proposed safeguards qualify as independent protection layers in a LOPA-style review.
Runaway review Reactor R-61 can overheat if monomer feed continues after cooling-water loss. The HAZOP lists several safeguards, but the risk owner wants LOPA clarity. Counting layers generously can make a weak scenario look controlled.
