Find the Vital Few in Claim Leakage
Use Pareto thinking to identify the few leakage drivers worth improving first.
A leakage list is not yet an improvement agenda. Claims leakage is preventable loss: overpayment, under-recovery, avoidable expense, rework, delay, or avoidable customer harm. The trap is to treat every leakage note as equally actionable. Count and weight Counting incidents tells you frequency. Weighting tells you effect. A category with five small misses may matter less than two high-dollar misses, but a category with forty low-dollar misses may reveal a weak control that deserves attention.
