Run the Blameless After-Action Review

Facilitate an after-action review that produces evidence-based improvements instead of blame.

After-action review protects learning while the memory is still alive. Start with intent Ask what we expected to happen. This prevents hindsight from rewriting the original plan. Reconstruct actual events Use timestamps, decisions, and message history. Specifics reduce defensiveness because the team is looking at shared evidence. Explain the gap Look for assumptions, role confusion, missing information, and pressure. Do not stop at the nearest person. Convert learning into commitments The review is not done until someone owns a change, a due date, and a test of whether the change worked.
