Blame frame The question is emotionally loaded and asks who failed. Who dropped the ball here? Naming a villain before naming the process gap. Your line The ownership point is fair. The process gap was that risk did not move from project review to launch review. The fix is a named risk owner before approval. It answers accountability without turning the exchange into a person hunt. Privacy boundary The question asks about a specific employee or confidential review.
