Design an approval stage with required evidence, decision fields, audit history, and exception handling.
A spend request tool needs manager approval for exceptions, but the team is tempted to use a free-text approval note. Treat approval as a controlled state transition with required evidence and durable decision history. A comment that says approved may look sufficient, but it does not enforce who approved, what evidence existed, or what should happen next. Step 1 Add a Pending Approval stage after triage and before fulfillment. The request should pause where judgment is required, not after fulfillment has already started.
