Make Near Misses Speakable

Create a kitchen response that makes safety near misses reportable while preserving accountability.

The move: make the first report safe and the correction serious. Kitchens run on urgency, hierarchy, and pride. Those forces can create excellent execution, but they can also make people hide uncertainty.
