Make QA Safe Enough to Tell the Truth

Create QA review conditions where agents can surface mistakes and improve faster.

The truth has to feel survivable. If agents only hear blame in QA, they will hide the messy details managers need. Safety Plus Standards Psychological safety is not permission to ignore expectations. It is permission to tell the truth about what blocked the expectation. That distinction matters on a regulated or high-volume floor. What Managers Signal Agents watch what happens after someone raises a defect. If the leader asks for evidence and fixes the system, candor increases.
