Compose upward feedback using permission, SBI, and a concrete request.
Pattern to address: risk gets dismissed quickly in staff meeting, then the team is asked later why the risk was not surfaced sooner. Permission -> Situation -> Behavior -> Impact -> Request The common shortcut is to name the pattern as a character flaw: "You do not listen to bad news."
