Build a 5 A's Action Plan

Use the 5 A's to create one nutrition action plan with support and arranged follow-up.

A client wants to bring lunch more often but leaves with only the vague instruction to meal prep. Use the 5 A's so the plan includes permission, assessment, advice, agreement, assistance, and follow-up. The common trap is treating 'I agree' as the end of counseling. A client can agree with the goal and still lack a trigger, support, or follow-up. Bring lunch more often this week.
