Control Existence Is Not Control Operation A common operational-risk mistake is treating 'we have a control' as the end of the assessment. COSO pushes a stronger view: controls live inside a system. They require a control environment, information flow, monitoring, and clear responsibilities. Without those pieces, the control can be present and ineffective at the same time.
