Evaluate Risk Before Averages
Evaluate agent failures by their effect on objectives, not just frequency.
Pass rate tells you how often something failed. Risk tells you whether the failure matters enough to change the decision. Consequence Start by naming the objective affected by the failure. Does it threaten money, safety, legal compliance, privacy, customer trust, or operational efficiency? The same hallucinated sentence has different significance in a brainstorming assistant than in a benefits-enrollment agent. Likelihood Likelihood still matters, but it is not the whole story. A rare failure in a high-impact workflow may need more attention than a frequent annoyance in a reversible workflow. Frequency should inform risk, not replace it.
