Refine the prompt, not just the output
Diagnose a weak model output and refine the prompt using concrete correction signals.
Do not just say "try again." Say what failed. A weak output usually points to one missing prompt ingredient: Goal: the model solved the wrong job. Context: it did not understand the situation or audience. Source: it used generic knowledge instead of your material. Constraints: it ignored length, tone, risk, or format. Example: it had no pattern for what good looks like. Turn that diagnosis into a refinement: "Keep X. Change Y. Use only Z. Return in format A. Avoid B."
