Turn CPQ defects into a Pareto fix list

Use Pareto analysis to prioritize CPQ defect fixes by concentration of rework causes.

Pareto analysis: group causes, count concentration, then prioritize recurrence reduction The trap is treating every ticket as a separate fix.
