The move: separate helping from owning. RACI is a responsibility map: Responsible, Accountable, Consulted, and Informed. In elder care, it is less about formal project management and more about preventing quiet drops between disciplines, shifts, and family communication. The most important distinction is Responsible versus Accountable. The responsible person may do the task.
