Test care-process changes with one PDSA cycle

The move: learn before you standardize. Plan-Do-Study-Act is a simple quality-improvement loop. It starts with a narrow question: What change are we testing, what do we predict, and how will we know? Then the team tries the change on a small scale, studies the evidence, and decides what to do next. This matters in elder care because good intentions can create operational harm. A new rounding rule may improve one resident's safety and overload the evening shift. A new family-update process may reduce complaints and duplicate documentation.
