Use SBAR to report a resident change of condition without burying the action needed.
The move: report the change, not the whole day. SBAR is a compact structure for urgent professional communication: Situation, Background, Assessment, and Recommendation. In elderly care management, it is especially useful when a resident is different from baseline and the team needs to decide what happens next. The power of SBAR is sequencing.
