Debug low recall in semantic search
Diagnose low recall by checking whether relevant sources are indexed, eligible, retrievable, and ranked high enough.
Recall miss Known-good sources are not appearing in top five. The issue may be corpus coverage, filters, query phrasing, top-k, or ranking. A fluent answer cannot cite a document retrieval never returns. The recall path Indexed -> Eligible -> Retrieved -> Ranked Low recall means a relevant document fails one of these gates.
