Use Five Whys without blaming the volunteer

Apply Five Whys to find a market operations root cause without turning it into blame.

The move: ask why the wrong action was easy. Five Whys is not interrogation. It is structured curiosity. Start with a specific problem: 'ADA aisle cones were six feet too narrow at 8:00.' Then ask why in a way that looks for conditions: instructions, materials, timing, layout, handoff, measurement, training, or ownership. Bad why 'Why did Sam place cones wrong?' This points at a person and invites defensiveness. Better why 'Why did the cone placement look correct from Sam's setup instructions?' This points at the information and cues Sam used.
