Walk a callback back to the process

Use Five Whys to move from a callback symptom to a process fix.

Callback A towel bar pulled loose eight days after install. The customer says, "I thought this was fixed." The goal is not to win the blame argument. It is to prevent the next loose fixture. Five Whys Symptom -> cause -> process fix Ask why until the answer points to a controllable process, not a vague character judgment. Blame Old wall, rough customer, cheap part.
