Treat repeated complaints as process signals

Use Five Whys to identify a process cause behind a recurring front-office complaint.

The reframe: Do not investigate the guest; investigate the process that failed the guest. A good Five Whys chain avoids character labels. 'Agent forgot' becomes 'no task was created from the comment field.' That wording matters because it points to a countermeasure. Stop when you can name a field, owner, checklist, cutoff, alert, or escalation rule. If you cannot name a countermeasure, you probably have not reached the process cause yet.
