Close the Defect Loop
Use the PDCA cycle to convert one housekeeping defect into a repeatable correction.
The move: fix the room, then fix the route the defect took through the system. Plan Name the defect in observable terms: location, standard missed, room status, and pressure at the time. Do not write "poor attention to detail" when the useful fact is "toothpaste streak visible from doorway after vanity wipe." Do Test one small control.
