Turn the post-incident review into engineering, not blame

Run a post-incident review that captures timeline, decisions, control failures, and durable remediation owners.

After-action reviews are part of response capability, not admin cleanup. When the team is tired, the easiest version of a review is a narrative: what happened, how hard everyone worked, and how the incident ended. That is emotionally understandable and operationally weak. A strong review asks sharper questions: where was detection late, where was evidence thin, where did approvals stall, which controls failed open, and which manual steps were more fragile than expected? Use four outputs. Timeline: what happened and when. Decision log: what calls were made, by whom, and with what evidence. Capability gaps: missing telemetry, unclear ownership, brittle…
