Just Culture Is Not No Accountability
Use just culture logic to respond fairly to safety events while preserving learning.
The outcome tells you severity; the behavior tells you accountability. The just-culture distinction A bad outcome can come from an honest slip, a drifted norm, or a conscious disregard of a substantial risk. Treating them the same damages safety. People hide slips if every error is punished, and they ignore standards if every action is excused.
