Make inspection a gate on every run
Run a pre-use ladder inspection that ends in use, correct-before-use, or remove-from-service.
The move: inspect to decide, not to reassure. A pre-use inspection should end in a decision. Start with the ladder, then the setup environment. Feet: are they present, clean, and gripping the surface? Rails: any split, bend, crack, corrosion, or modification? Rungs and steps: are they present, secure, clean, and slip-resistant? Locks and spreaders: do they fully engage? Accessories: are ropes, pulleys, hooks, levelers, and labels intact?
