Run a near-miss review without blame drift
Classify a laboratory near miss using Just Culture and lead a review that produces learning without excusing reckless behavior.
The move: classify the behavior before choosing the response. Just Culture prevents two common failures. One is blame drift: every event becomes a person problem. The other is accountability drift: every event becomes a system problem even when someone knowingly ignored a critical safeguard. Use three categories. Human error means an unintended slip; improve the system and console the person. At-risk behavior means a shortcut or drift became normal; coach the risk and redesign the work. Reckless behavior means conscious disregard of substantial risk; respond with accountability. Outcome severity tells you the stakes. Behavior category tells you the fair response.
