The lab logged 126 rejected specimens last month: clotted CBC 38, hemolyzed chemistry 31, unlabeled tube 19, wrong tube 16, insufficient volume 12, transport delay 10. The manager needs the first improvement target. Pareto analysis: rank defect categories by frequency or cost, calculate cumulative impact, then focus the next investigation on the largest useful category.
