Escalate risk with SBAR, not a long story
Use SBAR to make a mental-health clinic escalation concise, complete, and action-oriented.
The move: lead with the risk, then ask for the action. SBAR stands for Situation, Background, Assessment, Recommendation. It is useful in mental-health clinic management because many risk handoffs cross role boundaries. The person who first sees the signal may be a receptionist, care coordinator, biller, intake specialist, therapist, prescriber, or supervisor. Each person sees part of the picture.
