Build the SBAR before you call
Construct an SBAR escalation that includes a specific recommendation and urgency.
Tara needs to call the provider about Mr. Hale, who is newly confused, febrile, and lower than baseline blood pressure. SBAR turns scattered facts into Situation, Background, Assessment, and Recommendation. Common trap: giving every chart fact first and ending with "what do you want to do?" which forces the receiver to discover the urgency alone.
