Five Whys the fish loss

Use Five Whys to move from a visible aquatics problem to a process-level countermeasure.

Neon tetra losses spike after Thursday deliveries. The visible symptom is dead fish by Friday morning. Five Whys: keep asking evidence-based why questions until the cause points to a process countermeasure. The common trap is stopping at blame: "the vendor sent weak fish" or "the associate forgot." Those may be factors, but they do not produce a reliable fix by themselves. Why 1 Why were there losses Friday? The fish experienced stress after arrival.
