Create a lightweight FMEA for a refurbishment step and select the highest-priority control.

Refurbished blenders are passing visual inspection but 6% are returning again for motor failure within seven days of resale. FMEA: identify failure mode, effect, cause, detection, and control The common trap is to treat 'passed checklist' as proof of quality when the checklist does not detect the failure mode that matters.
