Pure recall What is a symptom? The visible effect or nonconformance. Contain it quickly, but do not confuse it with the cause. Do not blur these Hypothesis or evidence? Write hypotheses and evidence in separate columns. Meeting line We already know the cause: Alex missed the check. Say this That may be part of the chain. What process made the check depend on Alex remembering it? Stopping at the person who touched the defect last. It keeps the human action in view while moving toward a controllable process cause.
