Run a 12-minute After Action Review

Facilitate a short AAR that compares intent and reality before assigning improvements.

A beta invite was sent to 4,800 customers instead of the intended 480, and the first instinct is to find the person who clicked send. AAR sequence: Intended -> Actual -> Cause gap -> Change The common trap is asking "who approved this?" before the team has reconstructed the gap between plan and reality.
