The weekly customer escalation review starts late because nobody agrees which cases qualify. Support brings every angry email; Product wants only bug-related cases; Success wants renewal-risk accounts. The meeting needs a boundary before it needs another agenda. A fast SIPOC is built from the outside in: customer and output first, then the high-level process, then inputs and suppliers.
