DOWNTIME D D: Defects that create rework Defects are errors or missing inputs that create rework. Defects Errors, missing data, or bad outputs that force correction later in the value stream. O O: Overproduction before demand Overproduction is creating work before downstream demand exists. Overproduction Reports, batches, or outputs created before the downstream customer is ready or needs them. W W: Waiting between process steps Waiting is idle time between process steps or decisions.
