Why this matters A blameless postmortem is not a polite meeting where nobody is responsible. It is a structured learning process that assumes people acted with the information, tools, incentives, and pressures available at the time. The mechanism is truth access. People disclose more accurate timelines, assumptions, alarms, and near misses when they are not defending their job status. Accountability still exists, but it moves from personal blame to system improvement and clear ownership of fixes.
