Investigate Conditions, Not Just People
Choose an incident response that identifies system contributors and durable controls.
Incident investigation should explain how the event became possible, not only who was closest to the injury. OSHA safety programs emphasize finding and fixing hazards before recurrence. A blame-only conclusion often stops at the last action and misses the conditions that shaped it: jam frequency, tool availability, production pressure, guard design, lockout friction, training gaps, supervisor norms, and prior near misses. The mechanism of learning is causal depth. The deeper the contributing factors, the more durable the corrective actions can be.
