Run a Five Whys investigation that identifies a process fix instead of stopping at operator blame.
A tenant slipped at the east stairwell even though the crew salted the property at 5:50 a.m. Five Whys: move from event -> condition -> process cause -> countermeasure. The common shortcut is to stop at "the crew missed it." That may satisfy anger, but it does not explain why the miss was possible. Before Corrective action: remind crew to salt all stairs.
