Run PDCA on Install Defects
Use PDCA to reduce a recurring installation defect with a small measured change.
Do not coach a recurring defect until you have tested the process that creates it. The PDCA loop Plan names the defect and the proposed countermeasure. Do tests the countermeasure on a small slice of work. Check compares the result against the baseline. Act either standardizes the change or adjusts the next experiment.
