SBAR is a compression tool, not a script to sound formal. Situation Name the immediate issue in one sentence. Include the learner or client, the setting, and the concern that needs attention now. Background Add only context that changes the decision. Diagnosis, history, goals, risk notes, recent changes, or prior agreements belong here only if they help the receiver act. Assessment Say what you think the facts mean within your role.
