Root cause asks why it happened. Escape point asks why it reached you. You need both. Supplier corrective action often stops at the first satisfying explanation. A jam, wrong setup, worn tool, or missed instruction can explain occurrence. But if the defect moved through inspection, pack-out, release, or shipping, a detection control also failed. Occurrence cause This is the mechanism that created the nonconformance.
