Treat Near Misses as Control Tests
Use a near miss to identify which control failed or was missing.
Lucky is not a control. Read the miss as a system signal When a load rocks, a worker sidesteps a forklift, or a foot catches debris without injury, the warehouse just ran a control test. If the only lesson is 'be more careful,' the same exposure remains. Use four lines. Trigger names what changed: debris, congestion, wet floor, unstable pallet, blind corner. Exposure names who or what was in the hazard path. Weak control names what failed or was missing: no bin, unclear staging, no mirror, no inspection, no stop point.
